A Duty Holder Review (DHR) is a request from WorkSafe asking you to do an investigation, usually following an incident or accident. Generally if you've been asked to complete one it's at the lower end of the spectrum of seriousness of incidents, but nevertheless needs to be conducted and compliance with WorkSafe is not optional.
